1. Program Description
EasyDocForms sends automated SMS notifications to patients regarding their upcoming medical appointments. These messages may include links to secure intake forms, appointment confirmations, or pre-visit instructions.
2. Message Frequency
You will receive messages only when you have booked an appointment with a participating healthcare provider. Message frequency varies based on your appointment activity.
3. Cost
Message and data rates may apply to each message sent or received in connection with this service, as provided in your mobile telephone service rate plan.
4. How to Opt-Out
You can cancel the SMS service at any time. Just text "STOP" to the short code or number. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time or text "START", and we will start sending SMS messages to you again.
5. Support
If at any time you forget what keywords are supported, just text "HELP" to the number. After you send the SMS message "HELP" to us, we will respond with instructions on how to use our service as well as how to unsubscribe. For direct support, you can email us at [email protected].
6. Carrier Liability
Carriers (e.g., T-Mobile, AT&T, Verizon) are not liable for delayed or undelivered messages.
